Is it still necessary to be numbed with local anaesthetic? Will my dentist numb my gums before or after I’m sedated?

The drugs which are usually used for IV sedation are not painkillers (although some pain-killing drugs are occasionally added, see below for more info), but anti-anxiety drugs. While they relax you and make you forget what happens, you will still need to be numbed.

If you have a fear of injections, you will not be numbed until the IV sedation has fully kicked in. If you have a phobia of needles, you will very probably be relaxed enough not to care by this stage. Your dentist will then wait until the local anaesthetic has taken effect (i. e. until you’re numb) before starting on any procedure.

How is IV sedation given?

“Intravenous” means that the drug is put into a vein. An extremely thin needle is put into a vein close to the surface of the skin in either the arm or the back of your hand. This needle is wrapped up with a soft plastic tube. It makes the entry into the vein, then is slid out leaving the soft plastic tube in place. The drugs are put in through that tube (which is correctly referred to as an “indwelling catheter”, but more commonly known by the tradename of Venflon). The tube stays in place throughout the procedure.

The venflon to the right is a pinkie, which is one size bigger than the blue one that’s usually used for IV sedation in dentistry.

Throughout the procedure, your pulse and oxygen levels are measured using a “pulse oximeter”. This gadget clips onto a finger or an earlobe and measures pulse and oxygen saturation. It gives a useful early warning sign if you’re getting too low on oxygen, although if your dentist and the nurses are paying attention they should see it way before the machine does. Blood pressure before and after the procedure should be checked with a blood pressure measuring machine (a tongue-twister called “sphygmomanometer”, which for obvious reasons is referred to as “sphyg”).

One foreign patient walked into our Hus'hu Cliinic the other day. She is a middle- aged women from Phillipene living in Korea for years. Her looking was very similar with a korean. She wanted to have her back teeth removed and had a severe pain in lower right side of her mouth Her teeth health was not good. Cuase she was wearing denture on her upper teeth. Besides, she started loosing her teeth on the lower teeth.

As you can see on the X-ray photo, there is a intensive inflammation around back teeth of left side of her mouth. There were just two options to fix this problems. How does gum disease impact on our oral health??]

People usaully think cavaties are more severe and serious problems than gum diseases. Gum disease could cause more serious discomforts and problems as gum and gingivites are the pendants of the teeth. Plags and germs spreads along the root of the teeth. It melts away the whole gum structure and it subsides so the root part of the teeth would be exposed.

Gingivitis

When plaque is left around the teeth, it creates the right condition for bacteria to grow. These bacteria can irritate the gums and create gingivitis (inflammation of the gums). This makes the gums red and more likely to bleed easily. Gingivitis can easily be treated by a professional cleaning and polishing. Sometimes, I will recommend a medicated mouthwash and we will review how to brush and floss more effectively.

If gingivitis is left untreated, the inflammation will work its way down the tooth and form a “pocket”. This pocket is created because the bacterial are actually causing you to loose the bone around your teeth. We can measure the amount of bone loss around each tooth by doing Periodontal Pocket Measurements. Once these pockets have formed, it is much more difficult to clean the bacteria and you will need a scaling or deep cleaning of the pockets to get them clean. At this stage, gingivitis (inflammation of the gums) becomes periodontal disease (disease of the bone and supporting structures).

Periodontal Disease

Periodontal Disease is a silent disease and can slowly erode the bone and supporting structures of the mouth without any noticeable signs to the patient. If you wait too long, the teeth will actually become loose and by then, it is usually too late to reverse the process.

What is a Deep Cleaning?

As I mentioned above, gingivitis is easily treated with a professional cleaning and better home care of the patients. However, if you already have some pockets around your teeth, you will need a deep cleaning (root planning). This involves scraping the diseased tooth structure and bacteria from the root surfaces under the gums to help decrease the pockets around your teeth.

Does it hurt??

Depending on the depth of the pockets and the severity of the periodontal disease, I sometimes numb the area around the teeth I will be working on. This allows me to better clean the area and also keep you more comfortable. After the deep cleaning, I usually irrigate the pockets with and antimicrobial rinse to help flush out all the bacteria and tartar.

Follow Up

Deep cleaning is not something you want to do on a regular basis. That is why in our office we like to do 3-4 month follow-up cleanings after your deep cleaning. At each of these visits, we will redo the measurements of your pockets and make sure the disease is not getting worse. Typically, we do these 3-4 month cleanings for the first year after your deep cleaning. Once we have established that the disease is no longer progressing, we will put you back on a 6 month recall.

By ﬂossing your teeth daily, you increase the chance of keeping them for a lifetime and decrease the chance of getting gum disease.

Why should I floss?

Floss removes plaque and debris that adhere to teeth and gums in between teeth, polishes tooth surfaces and controls bad breath. By flossing your teeth daily, you increase the chances of keeping your teeth a lifetime and decrease your chance of having periodontal (gum) disease and tooth decay.

Flossing is the single most important weapon against plaque, perhaps more important than the toothbrush. A toothbrush cleans the tops and sides of your teeth. Dental floss cleans between them. Some people use waterpicks, but floss is the best choice. Many people just don't spend enough time flossing and many have never been taught to floss properly. When you visit your dentist or hygienist, ask to be shown.

Which type of floss should I use?

Dental floss comes in many forms: waxed and unwaxed, flavored and unflavored, wide and regular. Wide floss, or dental tape, may be helpful for people with a lot of bridge work. Tapes are usually recommended when the spaces between teeth are wide. They all clean and remove plaque about the same. Waxed floss might be easier to slide between tight teeth or tight restorations. However, the unwaxed floss makes a squeaking sound to let you know your teeth are clean. Bonded unwaxed floss does not fray as easily as regular unwaxed floss but does tear more than waxed floss.

How should I floss?

There are two flossing methods: the spool method and the loop method. The spool method is suited for those with manual dexterity. Take an 18-inch piece of floss and wind the bulk of the floss lightly around the middle finger. (Don't cut off your finger's circulation!) Wind the rest of the floss similarly around the same finger of the opposite hand. This finger takes up the floss as it becomes soiled or frayed. Maneuver the floss between teeth with your index fingers and thumbs. Don't pull it down hard against your gums or you will hurt them. Don't rub it side to side as if you're shining shoes. Bring the floss up and down several times, forming a "C" shape around the tooth and being sure to go below the gumline.

The loop method is suited for children or adults with less nimble hands, poor muscular coordination or arthritis. Take an 18-inch piece of floss and make it into a circle. Tie it securely with three knots. Place all of the fingers, except the thumb, within the loop. Use your index fingers to guide the floss through the lower teeth, and use your thumbs to guide the floss through the upper teeth, going below the gumline and forming a "C" on the side of the tooth.

How often should I floss?

At least once a day. To give your teeth a good flossing, spend at least two or three minutes.

Is it safe to use toothpicks?

In a pinch, toothpicks are effective at removing food between teeth, but for daily cleaning of plaque between teeth, floss is recommended. When you use a toothpick, don't press too hard, as you can break off the end and lodge it in your gums.

Do I need a waterpick (irrigating device)?

Don't use waterpicks as a substitute for brushing and flossing. But they are effective around orthodontic braces, which retain food in areas where a toothbrush cannot reach. However, they do not remove plaque. Waterpicks are frequently recommended by dentists for persons with gum disease; solutions containing antibacterial agents like chlorhexidine or tetracycline, available through a dentist's prescription, can be added to the reservoir in these cases.
