Hus'hu NEWYORK Dental Clinic - Periodental Disease


One foreign patient walked into our Hus'hu Cliinic the other day. She is a middle- aged women from Phillipene living in Korea for years. Her looking was very similar with a korean. She wanted to have her back teeth removed and had a severe pain in lower right side of her mouth Her teeth health was not good. Cuase she was wearing denture on her upper teeth. Besides, she started loosing her teeth on the lower teeth.


 
 As you can see on the X-ray photo, there is a intensive inflammation around back teeth of left side of her mouth. There were just two options to fix this problems. Dental implant and Partial denture were recommended for her. This is what happened to her during the extraction surgery. She just pushed her tooth with her tougn and it was pulled our itself. Dentist and medical team all were embarrassed about what's happening to her. But she relievely smiled cuase she didn't longer suffer from severe toothache.

So what cuased her to loose her teeth??  It was periodental disease

Periodontitis and gum disease are a progressive condition and the leading cause of tooth loss amongst adults in the developed world.  Periodontal disease occurs when the toxins found in plaque begin to irritate or inflame the gingiva (gum tissue).  The resulting bacterial infection often known as gingivitis, can eventually lead to the destruction of the gum tissue and underlying bone.  If periodontal disease is not treated, it can also lead to loose teeth or tooth loss.

 Common Signs &Symptoms
If you have any of these signs or symptoms, the advice of a general dentist or periodontist should be sought as soon as possible:
• Bleeding gums - Gums should never bleed, even when you brush vigorously or use dental floss.
• Loose teeth - Also caused by bone loss or weakened periodental fibers (fibers that support the tooth to the bone)
• New spacing between teeth - Caused by bone loose.
• Persistent bad breath - Caused by bacteria in the mouth.
• Pus around the teeth and gums - Sign that there is an infection present.
• Receding gums - Loss of gum around a tooth.
• Red and puffy gums - Gums should never be red or swollen.
• Tenderness or Discomfort - Plague, calculus, and bacteria irritate the gums and teeth
Treatment of Periodontal Disease
• Laser treatment – This can be used to reduce the size of the pockets between the teeth and the gums.
• Tissue &bone grafting – Where a considerable amount of bone or gum tissue has been destroyed, the dentist may elect to graft new tissue by inserting a membrane to stimulate tissue growth.
• Pocket elimination surgery – The dentist may choose to perform “flap surgery” to directly reduce the size of the gum pockets.

Wisdom Teeth are the third and final set of molars that most people get in their late teens or early twenties. Wisdom teeth that lean toward the second molars make those teeth more vulnerable to decay by entrapping plaque and debris. In addition, wisdom teeth can be entrapped completely within the soft tissue and/or the jawbone or only partially break through or erupt through the gum.
 
wisdom tooth that is fully erupted through the gum can be extracted as easily as any other tooth. However, a wisdom tooth that is underneath the gums and embedded in the jawbone will require an incision into the gums and then removal of the portion of bone that lies over the tooth. Oftentimes, for a tooth in this situation, the tooth will be extracted in small sections rather than removed in one piece to minimize the amount of bone that needs to be removed to get the tooth out.


The function of the wisdom teeth is also important as a one of moral teeth.  But why and when are they needed to be extracted??

1) First , when they are angled toward or away from the second molars or be angled inward or outward. Poor alignment of wisdom teeth can crowd or damage adjacent teeth, the jawbone as below.

 1. Cervical Abrasion
 Cervical Abrasion lesions are a common problems and cause the breakdown of tooth structure at the gum line over time.  In most cases, this is caused by excessive force used during brushing or using a hard-bristled toothbrush.
 Excessive force causes a recession of the gingiva which exposes part of the tooth called cementum that holds the tooth in the bone. That’s why brushing too hard can actually  wear down your tooth structure, resulting in increased sensitivity to hot and cold sensations.

 There are many kinds of reasons that causes the cervical abrasions.
 These can be categorized by several common reasons that cause this problems as below

How does gum disease impact on our oral health??]

People usaully think cavaties are more severe and serious problems than gum diseases. Gum disease could cause more serious discomforts and problems  as gum and gingivites are the pendants of the teeth. Plags and germs spreads along the root of the teeth. It melts away the whole gum structure and it subsides so the root part of the teeth would be exposed.

 Gingivitis  

  When plaque is left around the teeth, it creates the right condition for bacteria to grow. These bacteria can irritate the gums and create gingivitis (inflammation of the gums). This makes the gums red and more likely to bleed easily. Gingivitis can easily be treated by a professional cleaning and polishing. Sometimes, I will recommend a medicated mouthwash and we will review how to brush and floss more effectively.

 If gingivitis is left untreated, the inflammation will work its way down the tooth and form a “pocket”. This pocket is created because the bacterial are actually causing you to loose the bone around your teeth. We can measure the amount of bone loss around each tooth by doing Periodontal Pocket Measurements. Once these pockets have formed, it is much more difficult to clean the bacteria and you will need a scaling or deep cleaning of the pockets to get them clean. At this stage, gingivitis (inflammation of the gums) becomes periodontal disease (disease of the bone and supporting structures).

  Periodontal Disease 
 Periodontal Disease is a silent disease and can slowly erode the bone and supporting structures of the mouth without any noticeable signs to the patient. If you wait too long, the teeth will actually become loose and by then, it is usually too late to reverse the process.

  What is a Deep Cleaning?
 As I mentioned above, gingivitis is easily treated with a professional cleaning and better home care of the patients. However, if you already have some pockets around your teeth, you will need a deep cleaning (root planning). This involves scraping the diseased tooth structure and bacteria from the root surfaces under the gums to help decrease the pockets around your teeth. 


 Does it hurt?? 
  Depending on the depth of the pockets and the severity of the periodontal disease, I sometimes numb the area around the teeth I will be working on. This allows me to better clean the area and also keep you more comfortable. After the deep cleaning, I usually irrigate the pockets with and antimicrobial rinse to help flush out all the bacteria and tartar. 


  Follow Up  
 Deep cleaning is not something you want to do on a regular basis. That is why in our office we like to do 3-4 month follow-up cleanings after your deep cleaning. At each of these visits, we will redo the measurements of your pockets and make sure the disease is not getting worse. Typically, we do these 3-4 month cleanings for the first year after your deep cleaning. Once we have established that the disease is no longer progressing, we will put you back on a 6 month recall.
