Bone Grafting
In order for a dental implant to be successful the patient must have sufficient bone in the jaw to place into. Bone graft is the most common surgical dental procedure associated with dental implants. Usually the quality and quantity of bone in the jaw is reflective of how the bone healed after the tooth was removed. Thanks to advances in dental techniques and technology, the bone graft is very safe and effective in the dental implant restoration. When is bone grafting necessary?
1.The most common use for a Bone Grafting is during a Dental Implantation. Quite frequently, the dentist realizes that there isn’t enough bone to support the implant. A weak bone structure might be caused by factors such as gum disease, injury, trauma or missing teeth over a long period of time.
 2. In case of people whose jaw is too short or too narrow, bone should be added into the jaw before the implants can be placed.

The immediate implant means that the dental implants are inserted immediately after one or several dental extractions. With a immediate loaded dental implants procedure, phase 2 and 3 are done during the same day, This means that a patient has new teeth in place during the same trip the implant posts in placed. IV (Intravenous ) Sedation
Intravenous Conscious Sedation (aka “IV sedation”) is when a drug, usually of the anti-anxiety variety, is administered into the blood system during dental treatment.

What does it feel like? Will I be asleep?



A lot of dental offices and practices use terms such as “sleep dentistry” or “twilight sleep” when talking about IV sedation. This is confusing, because it suggests that IV sedation involves being put to sleep. These terms are more descriptive of deep sedation. Deep sedation isn’t commonly used (in the U.K. at least), and is classified as general anaesthesia (even though sedation occurs on a continuum).
In reality, you remain conscious during conscious IV sedation. You will also be able to understand and respond to requests from your dentist.
However, you may not remember much (or anything at all) about what went on because of two things:

  1. IV sedation induces a state of deep relaxation and a feeling of not being bothered by what’s going on
  2. the drugs used for IV sedation produce either partial or full memory loss (amnesia) for the period of time when the drug first kicks in until it wears off. As a result, time will appear to pass very quickly and you will not recall much of what happened. Many people remember nothing at all. So it may, indeed, appear as if you were “asleep” during the procedure 




Is it still necessary to be numbed with local anaesthetic? Will my dentist numb my gums before or after I’m sedated?

The drugs which are usually used for IV sedation are not painkillers (although some pain-killing drugs are occasionally added, see below for more info), but anti-anxiety drugs. While they relax you and make you forget what happens, you will still need to be numbed.
If you have a fear of injections, you will not be numbed until the IV sedation has fully kicked in. If you have a phobia of needles, you will very probably be relaxed enough not to care by this stage. Your dentist will then wait until the local anaesthetic has taken effect (i. e. until you’re numb) before starting on any procedure.

                                    
How is IV sedation given?
“Intravenous” means that the drug is put into a vein. An extremely thin needle is put into a vein close to the surface of the skin in either the arm or the back of your hand. This needle is wrapped up with a soft plastic tube. It makes the entry into the vein, then is slid out leaving the soft plastic tube in place. The drugs are put in through that tube (which is correctly referred to as an “indwelling catheter”, but more commonly known by the tradename of Venflon). The tube stays in place throughout the procedure.
 The venflon to the right is a pinkie, which is one size bigger than the blue one that’s usually used for IV sedation in dentistry.
 Throughout the procedure, your pulse and oxygen levels are measured using a “pulse oximeter”. This gadget clips onto a finger or an earlobe and measures pulse and oxygen saturation. It gives a useful early warning sign if you’re getting too low on oxygen, although if your dentist and the nurses are paying attention they should see it way before the machine does. Blood pressure before and after the procedure should be checked with a blood pressure measuring machine (a tongue-twister called “sphygmomanometer”, which for obvious reasons is referred to as “sphyg”).

1. Flapless Implant

Flapless dental implant surgery is the type of implant surgery where incisions are not utilized during the surgery. The use of CT scans and computer guided implant placement techniques has allowed us to be able to place dental implants directly through the gums without the need to visualize the bone. This is because the CT scan gives an accurate three dimensional representation of the bone and surrounding structures. Flapless dental implant surgery allows for faster healing, less inflammation and less post-op discomfort and pain. In some cases, flapless surgery may not be possible and traditional techniques may be indicated.

2.  One - Day Implant

One day implants (immediate load implants) it is now possible to walk away with a temporary crown firmly in place on the same day the pin has been implanted into your jaw. Not only has new technology reduced the time it takes to perform an implant but implant procedures can now be performed with minimal invasive surgery. Within one month of the original surgery the permanent crown can be fixed into place and the tooth implant is complete.

3. Immediate Implant

One of the advantages of immediate implantation is that post-extraction alveolar process restoration is reduced, thus affording improved functional and esthetic results. Another advantage is represented by shortening in treatment time, since with immediate placement it is not necessary to wait 6-9months for healing and bone neoformation of the socket bed to take place. Patient acceptance of this advantage is good, and psychological stress is avoided by suppressing the need for repeat surgery for implantation.
